Hi Gavin, Being new to this board. I am wondering is there a usual 1st line of chemo doctors use with metastatic cholangiocarcinoma with mets to the liver? Is it best to jum right into a clinical trial? my father na law already had Gem/Cis and Radiation after resection a year and half ago

Re: Targeted Therapy of Advanced Gallbladder Cancer and Cholangiocarcinoma

Hi,
Thanks Gavin for the info.
Rebecca ,if your father in law has liver metastasizes, I will get a 2nd opinion from the liver surgeon and also from an interventional radiologist to see whether the tumors can be removed first ,then will talk with the medical oncologist for adjuvant chemotherapy or targeted therapy. MASS GENERAL,JOHN HOPKINS SLOAN-Kettering  are great hospitals just to name a few near you.

First line chemo therapy include. GEM/CIS, GEMOX, 5FU/CIS,FOLFOX among others, second line including GTX,TARCEVA+Avastin, Sorafinib ,Rituximab etc.

Below is a link about systemic chemotherapy and targeted therapy in general, so you can have some ideas.

Clinical trials are worth it depends on each individual case and the availablity and suitability  to the patient, it is best to ask referral from your current oncologist if he/she opens to the idea.
Please check out the clinical trials on our web site under the cholangiocarcinoma on the top , open and click clinical trials and you will have some idea whether your father can go .

God bless.

You're welcome Percy.

Rebecca, have you spoken with your onc about a trial for your FIL and if so, what did they say about trials? And how did your dad get on with the Gem/Cis, did he tolerate it well?

Here is the link that Percy mentions -

It has a video webinar in it that is well worth watching all about trials etc.

Best wishes,

Gavin

Re: Targeted Therapy of Advanced Gallbladder Cancer and Cholangiocarcinoma

Hi Gavin and everyone else. I ve read this post and  it helped me do tons of research.
We actually sent my father n law to Croatia while he was still feeling great even though his tumours grew significantly we felt like it was important to see his family.
He is getting 2 opinions on the next step in a week or so and unfortunately I cant see doctors with out him.
I will definitely get back on and report. Dr Schwartz his surgeon would like to see the tumours stable for some period of time before resecting and does not feel it is ammenable to a stereotactic RT at this time.

I hope every one is doing ok. Thank you so much for your support

Hi Rebecca, so good to see you again! What a visit that must have been for your FIL. I am very glad to hear he is getting 2 opinions! Please let us know the outcome of the 2 visits. He must be quite a man to make a trip like that. Looking forward to reading about what comes next and sending a ton of best wishes!
